Replacement Policy:

Guarantee:  All merchandise purchased from Flat Screen Clean™ has a replacement guarantee for any damaged products within 14 days from the date of purchase.  

When your order arrives, please inspect the package for any damage that may have occurred during shipment.  It is normal for the package to show some wear, however, if the product itself is damaged during shipment, please contact us immediately at customer service.

9. (e.g. shipping, product malfunction/error)?
10. What part of the product was damaged?

Please allow 2 business days for your replacement to be shipped.

THIS FORM MUST BE FILLED OUT IN ENTIRITY IN ORDER TO RECEIVE A RETURN AUTHORIZATION NUMBER!

Once you receive a return authorization number through e-mail or telephone, please do the following to ensure a refund:

1. In a standard envelope, send only the clear cap from  the product.
2. On the envelope you MUST have the return authorization       number. 
3. Please include your name and address in the upper left  corner of the envelope and a stamp in the right corner.
4. When addressing the envelope to us please make it look like the following EXAMPLE:

The CLEAR CAP is the ONLY part of the product that is enclosed in the envelope.  DO NOT SEND THE BOTTLE BACK!  Dispose the liquid and recycle the bottle.
